Thanks for choosing Dr. Yagnik and Baptist Health Orthopedic Institute for your Orthopedic care. In effort to have a smooth visit we ask that you read the following information below.

When to Arrive

  • Please arrive 15 - 30 minutes before your scheduled appointment.
  • Self-parking is available at all locations
  • Free Valet Parking is offered & encouraged at the West Kendall Baptist Hospital & Doctors Hospital Locations

What to Bring with You

  • Picture ID
  • Insurance card
  • An accurate and updated list of all your current medications or your physical medications
  • Imaging – If you have done any advanced imaging (CT or MRI), please bring a copy of the Images on a CD and a copy of the radiology report. Dr. Yagnik prefers to view and interpret all imaging himself, so it is very important to bring a copy of the actual images, not just the report.
  • Knee Patients – please bring or wear shorts, so we can examine your knee
  • Shoulder/Elbow Patients – Please bring or wear a short sleeve shirt or tank top, so we can examine your shoulder/elbow.
  • Second Opinions – If you are coming for a second opinion or if you have had prior surgery, please come prepared with as much information as possible to help us better assist you. Items to consider bringing include prior Imaging (X-rays, CT or MRI), Operative Report from you prior procedure, Medical Records/Office Notes related to your prior surgery, treatment or hospitalization and any information regarding future treatment that has been recommended to you.
  • International Patients – Baptist Health South Florida has an entire department dedicated to assisting international patients. Please contact the Baptist Health South Florida International Department to help facilitate your visit.

What Expect During Your Visit

  • X-Rays – Dr. Yagnik will order and perform special x-rays in the office. We order specialized views that help us to better visualize your anatomy.
  • You will meet and interact with several members of Dr. Yagnik’s team during the visit, including X-ray Technicians, Athletic Trainers/Medical Assistants and Front Desk Staff. All of these staff members are essential to the process and will help you to have a thorough and efficient consultation. Please treat them kindly.
  • We do everything we can to make your visit as smooth and efficient as possible. The whole process of the visit generally takes about an hour, sometimes more depending on the complexity of your problem. Please be patient as we care for you.
